Coil closure of lumbar artery pseudoaneurysm: An unusual complication associated with anomalous left circumflex artery primary percutaneous coronary intervention

چکیده
The presence of anomalous coronary arteries is observed in approximately 1% of patients undergoing coronary angiography (CAG). In these patients, identification of the stenotic ostium and revascularization is difficult, particularly in the emergency setting of primary percutaneous coronary intervention (PCI). Among the congenital coronary anomalies, separate anomalous origin of all the coronary arteries from the right sinus of Valsalva is very uncommon. Failure to identify the anomalous origin of coronary arteries can lead to in correct diagnosis and prolonged procedures, which can result in serious complications, especially in the setting of acute myocardial infarction. A male patient 60 years old presented with acute inferior and posterior wall myocardial infarction with postmyocardial infarction (MI) angina. On evaluation his ECG revealed sinus rhythm, q II,III, aVF, Tall T in V1, V2. Two dimensional (2D) Echo showed regional wall motion abnormalities of the inferior & posterior walls, a moderate degree of MR, Gr II diastolic dysfunction, and an EF of 47% with good RV function. Biochemical values were within normal limits. TROP Twas positive. In view of rest angina, the patient was taken for early revascularization. His coronary angiogram revealed dominant left system and left circumflex artery (LCX) with total occlusion, arising from the right sinus (Fig. 1). The LCX was engaged with an AL1 catheter (Amplatz

Congenital Absence of Left Circumflex Coronary Artery
Congenital absence of left circumflex artery is a rare congenital anomaly of the coronary arteries. The prevalence of the anomaly in different studies ranges from 0.6% to 1.3%. Of these, 80% are benign and asymptomatic and 20% are clinically important. We report a 56-year-old man presented with acute resting chest pain who was diagnosed as having acute anterolateral infarction accompanied by el...
متن کاملClinical Implications of Congenital Absence of Circumflex Coronary Artery
Introduction: Coronary artery anomalies are rare clinical entities reported in 0.6% to 5.6% of diagnostic coronary angiographies. Anomalous origins of coronary arteries from distal segments are rarely reported. Presented herein is a coronary anomaly in which the circumflex artery (CX) originated as a continuation of the posterolateral branch of the right coronary artery (RCA) w...
متن کاملPrimary Percutaneous Coronary Intervention Angioplasty of Occluded Twin Circumflex Coronary Artery in a Patient of Acute Inferior Wall Myocardial Infarction: A Rare Anomaly
Anomalies of the coronary arteries are reported in 1-2% of patients among diagnostic angiogram. Dual origin of a circumflex from both sinuses is extremely rare among them. We report a case of a patient who underwent primary percutaneous coronary intervention for acute inferior wall myocardial infarction where left coronary injection demonstrated normal obtuse marginal and right coronary injecti...
متن کاملAnomalous origin of the circumflex artery and patent foramen ovale: a rare cause of myocardial ischaemia after percutaneous closure of the defect.
A 35 year old man with a history of transient ischaemic attack and atrial septal aneurysm underwent percutaneous closure of a patent foramen ovale (PFO) with a transcatheter device. After the procedure the patient developed effort angina not present previously. Transoesophageal echocardiography confirmed the absence of residual shunt but showed an abnormal linear image running behind the aortic...
متن کاملTransradial Percutaneous Coronary Intervention in a Patient with a Rare Coronary Anomaly: Twin Circumflex Arteries.
UNLABELLED Coronary artery anomalies are rare in population and most of them are found incidentally during coronary angiography. Percutaneous treatment of critical lesions on anomalous arteries may lead to difficulties due to their abnormal origin or course. Herein, we report a 65-year-old male patient presented with recent onset chest pain. Electrocardiogram and transthoracic echocardiography ...
